Your First Visit

Patient Forms

New patient forms may be downloaded here and brought with you for your first appointment. These forms are also available at our office and may be completed upon arrival. Please bring any insurance cards as well as an ID with you. If you lack insurance or your insurance doesn’t cover chiropractic care, we have an affordable care plan for patients paying out of pocket.

Consultation

Next, you will have a consultation with Dr. Mattern to discuss your health-related problems, concerns, and potential chiropractic treatment options. This initial visit is designed for the doctor to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.

Examination

Following your consultation, Dr. Mattern will perform a complete chiropractic examination, which will include neurological, orthopedic, postural, and other physical tests. This will help the doctor gain more insight into what is physically going on with your body. Please note, treatment will not be performed without your consent.

Report of Findings

After reviewing your health history, goals, examining your spine, and movement Dr. Mattern will discuss his impressions and recommendations. He will notify you if your condition requires consultation with your primary care provider or other providers prior to chiropractic care. Above all we want to make sure you are in the right office.

Treatment

Next, you can choose to experience your first treatment. This may include spinal adjustments, physical therapy, soft tissue massage, and/or myofascial therapy. Treatment is interactive so you can express concerns or questions about the different styles of treatment at any time.

Wellness Program

In addition to chiropractic care, the doctor may suggest a wellness program to incorporate outside of treatment to speed up your recovery. This may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises. As with all our patients we want you to achieve rapid pain relief and return to full activity.
